My wife and I have a joint account, but only my name appears on the form. Can she get the insurance?

Sometimes the forms only name the first person listed on the account, but either one of you is eligible. If you prefer to have the insurance on your wife, please contact us to request an additional enrollment form.

Can both my spouse and I get the complimentary coverage?

In most cases the offer for the complimentary coverage is limited to one person per joint account.

My wife has her own account at the bank. Why didn't she receive an enrollment form?

Often only one offer is mailed per household. You may contact us to request an additional enrollment form. Please note, some offers are only made to checking account holders and savings accounts may not qualify.

Why did this come in my husband's name? I'm just as much a customer as he is.

These offers are sometimes mailed with just one joint account holder's name on them, usually the first name listed on the account. If you would rather have the coverage for yourself, please contact us to request an additional enrollment form.

My wife and I are signing up for the Family Plan and both of our names are listed on the form. Which birthday should I put in the blank where it asks for the insured's date of birth?

Circle the name of the person you want to be the primary insured. That person's birthdate should be written on that blank. The primary insured should also be the person who signs the form.
